Connections Physical Therapy Privacy Policy

Effective date: 10/31/2026

This policy explains how Connections Physical Therapy, Inc. and its clinics, including Holmes Physical Therapy, Live Free Physical Therapy and Medfield Orthopedic & Sports Therapy (“Connections PT”, “we”, “us”), collect, use and protect information you give us through this website, our forms, live chat, email and text messages.

1. Health information

Health information we create or receive while caring for you is protected by HIPAA and handled as described in our Notice of Privacy Practices, which is available at every clinic and on request. Where this policy and the Notice differ about your health information, the Notice applies.

2. Information we collect

We collect your name, email address, phone number and other details you give us when you request an appointment, fill out a form, register as a patient, use live chat, sign up for messages, or contact us.

3. How we use it

We use your information to:

  • Schedule appointments and respond to your questions
  • Send appointment confirmations, reminders and schedule changes, including clinic closures and delayed openings
  • Handle registration, insurance, billing and other administrative matters
  • Run surveys and ask for ratings and reviews
  • Follow up after you contact us

4. How we protect it

Our website is scanned regularly for security holes and known vulnerabilities. Your information is stored on secured systems that only a limited number of authorized people can access, and they are required to keep it confidential. Information you send through our website is encrypted using Secure Socket Layer (SSL) technology. Online bill payments are processed by a payment provider and card details are not stored on our servers.

5. Cookies

Our website uses cookies to remember your preferences and to understand how visitors use the site so we can improve it. You can set your browser to warn you before a cookie is placed or to turn cookies off. If you turn them off, some features of the site may not work properly.

6. Google

We use Google Analytics to understand how visitors use our website. You can opt out using the Google Analytics Opt-out Browser Add-on.

Google, as a third-party vendor, may use cookies to show our ads on other websites based on your visits to ours. You can opt out of personalized ads in Google Ad Settings or through the Network Advertising Initiative opt-out page.

7. Do Not Track

Some browsers send a “Do Not Track” signal. There is no common standard for how websites should respond to it, so our website does not currently change its behavior when it receives one. You can limit tracking using the cookie and opt-out options above.

8. Sharing your information

We do not sell, trade or rent your personal information. We share it only with service providers who help us run our website and serve patients, such as our scheduling, phone, email and messaging providers, and they are required to keep it confidential and use it only for the services they provide to us. We may also disclose information when the law requires it or to protect the rights, property or safety of our patients, staff or others.

9. Text messaging

If you agree to receive text messages, we may text you about appointments, schedule changes such as clinic closures and delayed openings, registration, insurance, billing and other administrative matters. Messages are sent by our authorized service providers, including Shasta Health and Amazon Web Services. Your mobile number, opt-in information and consent records will not be sold, rented or shared with third parties or affiliates for their own marketing or promotional purposes.

Reply STOP to any message to opt out, or HELP for help. Message and data rates may apply. Agreeing to receive texts is not a condition of receiving care. Text messages are not encrypted, so please do not send detailed medical or financial information by text. See our SMS Terms & Conditions for details.

10. Email

You can unsubscribe from our emails at any time using the link at the bottom of each email, or by contacting your clinic. We will remove you promptly. Unsubscribing from marketing emails does not stop messages about your appointments or account.

11. Children

Our website is not directed to children under 13, and we do not knowingly collect personal information online from children under 13. Information about patients under 18 is provided by a parent or guardian.

12. If there is a data breach

If your personal information is involved in a security breach, we will notify you as required by law.

13. Changes to this policy

We may update this policy. Changes are posted on this page with a new effective date.
